Grounded in faith and guided by Colossians 3:23, we are committed to serving others with integrity, compassion, and excellence while working collaboratively with individuals, families, Support Coordinators, providers, and other members of the support team.
2. How Does the Placement Process Work?
Our placement process begins with a referral or inquiry. We take time to understand each individual's needs, preferences, goals, support requirements, and preferred living environment.
Potential homes are carefully considered based on compatibility, location, accessibility, and individual needs. Whenever appropriate, individuals have an opportunity to visit and become familiar with the home and provider before making a decision.
Our goal is not simply to find an available placement, but to help identify a home where each individual can feel safe, comfortable, respected, and included.
3. Are Your Providers Licensed and Qualified?
Profound Performance LLC is licensed by the Virginia Department of Behavioral Health and Developmental Services (DBHDS) to provide licensed services.
Sponsored Residential Providers and Direct Support Staff must meet applicable agency, DBHDS, and Virginia Medicaid requirements, including required screenings, training, qualifications, and competencies before providing services.
Sponsored providers operate under Profound Performance LLC's licensed Sponsored Residential program and must meet all applicable requirements before an individual is placed in their home.
4. What Areas Do You Serve?
Profound Performance LLC is based in Danville, Virginia, and provides Sponsored Residential Services in areas of Virginia where qualified and compatible sponsored homes are available.
Because placement is based on each individual's needs, preferences, and home availability, we encourage families and Support Coordinators to contact us regarding opportunities in their preferred area.
5. How Do I Request a Placement?
Individuals, families, and Support Coordinators may begin the process by submitting our Sponsored Residential Placement Inquiry/Referral Form.
The form collects basic information needed to help us understand the individual's service and placement needs. After receiving the inquiry, a member of the Profound Performance team will follow up to discuss availability and the next steps in the referral and screening process.
6. How Do I Become a Sponsored Residential Provider?
Profound Performance LLC welcomes compassionate, qualified individuals interested in becoming Sponsored Residential Providers.
We seek individuals who can provide a safe, stable, respectful, and welcoming home while supporting each person's health, safety, choices, independence, relationships, and quality of life.
Potential providers complete an application, screening and interview process, along with applicable background checks, training, competency requirements, documentation, and home reviews before providing services through our agency.
